• Disrespectful comments about patient

• Clinical decisions attempted outside official system

• Unclear membership/access control

• “Speed culture,” lack of approved digital workflow

• No group governance (membership, rules, logging)

• Low awareness of privacy and professionalism

• Ambiguous roles (who can authorize medication changes)

Immediate corrective actions (containment)

• delete/revoke shared files, stop screenshots

• remind rules, reduce membership

• move clinical decisions to EMR/official channel

• document incident objectively

Preventive actions (system)

• adopt an approved communication platform / SOP

• micro-training on digital professionalism

• audit trail and periodic review

• “speak-up” pathway without punishment
