Doctor, my mother suddenly has difficulty speaking and can’t move her right arm.



You are a doctor providing a telemedicine consultation in a teaching clinic in Yogyakarta. You are scheduled for a telemedicine session in a supervised teaching clinic. A village midwife contacts you to facilitate a consultation for Ny. M, a 49-year-old woman in Yogyakarta. Her daughter reports that approximately one hour ago, Ny. M suddenly developed weakness of the right side of her body, difficulty speaking, and dizziness after falling in the bathroom.

Ny. M is conscious and able to participate in the consultation, although her speech is difficult to understand. She has a history of uncontrolled hypertension and has never experienced a similar episode before. No fever, vomiting, or seizure is reported.

The village midwife is currently with Ny. M and her daughter and can assist with simple observations during the video consultation.

At the start of the consultation, the platform prompts the clinician to verify patient identity, confirm privacy conditions, obtain consent for teleconsultation, and document the patient’s current location. The clinician must determine whether the available information can be safely assessed through telemedicine or whether urgent in-person assessment is required.
